Hammersmith Conservative Association was formed on the 1st of January 2007. The Boundary Commission redrew the constituencies of West London for the 2010 general election.
The constituency boundary runs from the Lillie Road and the River in the South, to the Harrow Road in the North, and from the Holland Road in the East to the Old Oak Road, The Goldhawk Road and Emlyn Road in the West.
The constituency is made up of fourteen wards:

- Avonmore
- Brook Green
- College Park and Old Oak
- Coningham
- Fulham Reach
- Grove
- Hammersmith Broadway
- Ravenscourt
- Shepherds Bush Green
- Wendell Park
- West Kensington
- White City
- Wormholt
In late 2022, the Boundary Commission recommended significant changes to take effect at the next election that will split the seat three ways. Parts will join with Acton and Fulham, whilst most wards will form the core of a new Hammersmith and Chiswick parliamentary seat.
